I was told this over the phone, But I have not tested it.
key the following code on your guitar at the title screen
it is suppose to unlock everything
Blue
Yellow
Orange
Red

I play on Expert on both and have beaten both but...I really couldn't tell you how I do it, I just do.
It's something you grow onto, you'll just know.
But yes, you do need big hands.

I know how you feel. I could do the speed that Hard is on, if it didn't utilize the fifth button. I always lose track of were my hand is on the guitar and have to look, and then I get lost on where I am in the music. If there was a medium with the rhythm that hard had, I'd be able to do it, I just can't mentally keep track of where my hand is.
@noz3001: Medium uses four buttons... Hard/Expert uses five.
@Butters: You don't need big hands if your playing Rock Band's guitar since it has the smaller buttons at the base of the neck.

Play it non stop on medium and when you get bored on medium, do hard. That's what I did and I can beat TTFAF on Hard.

Its called muscle memory, you gain this skill by playing the game alot, and learning where your fingers should be.
