Usually means the file is from another location, but was created, in that new location, on that given date.
For example, you have files stored on a CD, each have their own sets of dates. Copy one to your desktop and majority of the time the modified date stays the same from when it was last edited on the old location.

Continuing last post.
Because copying a file from a CD/The Web is considered Recreating the file, and updates the created field.
