Is it possible to set an exception handler in your driver and set breakpoints on usermode addresses after attaching to them using KeStackAttachProcess?

Hardware breakpoints are thread specific so you need to execute the setting of the dr# registers from inside the context of each thread. Try an apc, or when an interrupt happens
