If the spy is disguised as your team member, there is no way to tell it's not them except for 2 things.
1. They will never fire/use weapons
2. Sometimes they won't be the same class as said person.
i.e: While I'm spychecking, i'll see some demo from the same team coming, but when i put my mouse over him, he has my name, so he's a spy.
And if they're disguised as a friendly spy, they will be "disguised" as a random(i think) class of the opposite team.

When you're a medic, some people would walk up to you for a heal. Shoot them first with your syringe gun. If it just passes through him, he's not a spy. Just shoot of your teammates with your syringe gun if they ask for a heal.
